Use of Facilities

For outside community groups interested in renting CHS facilities, please go to www.fuhsd.org and click on the “Use of Facilities” link on the left hand side of the main page – it will take you to the application and samples of insurance.    Please note that the application is two pages and requires a signature on both pages.  Once you have everything filled out and signed, you can email, mail or hand carry this into Lisa Balga in the ASB Office.    After receiving the application the process of approval will start.    Please feel free to contact me if you have any questions or concerns.
